Corrections and disputes
Last updated 6 October 2026
Records in the Index name real organizations and people. This page explains how to challenge a record and what we do when you do.
Who can raise a dispute
Anyone can ask for a record to be corrected, updated or reviewed, including a person or organization named in it. There is no charge, and the Index never accepts payment to change a record, its evidence status or its classification.
How to raise one
Use the correction form. Give the record identifier or its web address, say what you believe is wrong, and link the evidence that supports your view. A name and an email address are optional, but without an email address we cannot reply to you.
If you cannot use the form, email data@aiincidentindex.org with the same details.
How we review it
An editor reviews the correction against the evidence you cite and the sources already listed on the record. Reviews follow the published methodology, including the rule that community material alone never qualifies an incident as confirmed.
- We aim to acknowledge a dispute within 5 working days.
- We aim to tell you the outcome within 30 days of receiving it.
What can happen to a record
- Corrected: if the correction is substantiated, the record is updated. Where earlier reporting was materially wrong, the evidence status becomes Corrected.
- Annotated or marked Disputed: where a named party contests material facts and the evidence does not settle the matter, the record can carry the Disputed status and the party's response alongside the original sources.
- Removed: a record is removed only if it does not meet the inclusion standard in the methodology, for example because it is not a documented incident or duplicates another record. A record is not removed only because it is unwelcome to the organization it names.
- Unchanged: if the evidence does not support a change, the record stays as it is and we explain why.
